There are probably many genotypes that predispose someone to an ADHD phenotype.

First of all, these is no perfect way to define ADHD. That's the case for pretty much any psych disorder. Whatever definition you use (DSM, ICD, your own system of "AD(H)D") will be somewhat circular and arbitrary.

The general consensus is that ADHD is a neurodevelopmental disorder with strong genetic components, which leads to altered neurological functioning. Maybe in the future, we'll be able to pinpoint specific genes. Or more importantly, maybe we'll be able to directly "look inside" the brain to determine the exact brain pathways that are underlying the ADHD symptoms. For instance, maybe one person has defective genes for dopamine-producing, and we can directly monitor the amount of dopamine in the brain. Another person might be producing a normal amount of dopamine, but maybe they have overactive dopamine transporters. Another person might turn out to only have an issue with the GABA system, indicating an anxiety diagnosis rather than an ADHD diagnosis.

But that's just future speculation. What we're stuck with right now is using symptoms to make a best guess for psych diagnosis.
